Cognito Form Buildout

Create a polished and organized client experience with Cognito Form Services tailored to your business. Whether you need to refresh an existing form or have one built from the ground up, choose the option that best fits your needs.

Option 1: Form Design

Give your existing Cognito form a professional, branded look using your provided logo, colors, fonts, images, and other brand elements.

Form creation, restructuring, new fields, and functionality changes are not included.

Option 2: Form Creation

Have a new form created inside your Cognito Forms account using the content and requirements you provide. This may include applicable fields, sections, document uploads, electronic signatures, acknowledgments, and submission confirmations.

Custom design and branding are not included.

Option 3: Form Buildout (Design + Form Creation)

Get the complete experience with a form created from the ground up and professionally customized to match your brand. This option combines Form Design and Form Creation into one complete service.

How It Works

1. Select your service option and preferred turnaround time.

2. Complete the Graphic Design Inquiry Form after purchase with your project details and materials.

3. Your Cognito Forms project will be completed based on your selected service and delivered for review.

Please Note

• An active Cognito Forms account is required. Clients are responsible for all account, subscription, upgrade, and platform related fees.

• Workflow automation, advanced integrations, and third-party software setup are not included.

• Clients must provide all required content, including questions, agreements, policies, disclosures, and compliance language, and are responsible for reviewing and approving the final form.

Once you have paid click HERE to complete the design inquiry form. This form must be completed within 2 hours. Failure to do so could delay production of your digital product(s).

Digital Product Policy: This is a digital product. No physical product will be delivered.

Turnaround Time: 4-7 business days and begins once all required information has been received. Business days exclude weekends and holidays.

Expedited Turnaround: Available for an additional fee and delivered within 24–48 hours once all required information has been received. All materials must be submitted within 1 hour of purchase with timely communication. Failure to do so may result in forfeiture of the expedited fee and the order reverting to standard turnaround.

Revision Limit: Includes up to two complimentary revisions within scope. Revisions must be requested within 24 hours of delivery or will be forfeited. Additional revisions or requests outside of scope are $25 each.

Revision Time: Each revision may take up to 24 hours and is not included in the original delivery timeframe.

Communication & Inactivity: Timely communication and submission of required materials are required. Failure to respond may result in delays, project inactivity, or forfeiture of services per our full policy.

Delivery: Delivery is considered complete once final files have been sent.
