• Client information includes full legal name, date of birth, Social Security number, email address, phone number, current home address, filing status, occupation or employer name, and preferred method of contact

• Spouse information, if applicable, includes spouse’s full legal name, date of birth, Social Security number, occupation or employer, email address, and phone number

• Dependent information, if applicable, includes dependent full name, relationship to the client, date of birth, Social Security number, residency status, student status, and childcare expense information. Fields may be repeated for multiple dependents

• Income information includes W-2 income, self-employed or 1099 income, business name if applicable, additional income sources such as rental, investment, retirement, or unemployment income, and additional income notes

• Deductions and credits include education expenses, child tax and childcare credits, student loan interest, medical expenses, charitable contributions, homeownership expenses, and retirement contributions

• Banking and refund information includes preferred refund method, bank name, routing number, and account number

• The document upload section allows secure uploads for W-2s, 1099s, prior-year tax returns, photo identification, Social Security cards if required, and additional supporting documents

• Compliance and authorization includes tax preparation consent, electronic signature, submission date, and acknowledgment of accuracy and responsibility

Tax Waitlist Form Buildout – What’s Included

Our Tax Waitlist Form buildouts are designed to collect basic client interest information only, making them ideal for pre-season enrollment, capacity management, and lead generation

• Client contact information includes full legal name, email address, phone number, and city/state of residence

• Filing type selection includes options such as single, married filing jointly, married filing separately, head of household, or other applicable filing categories

• Service interest selection allows clients to indicate the type of tax service they are seeking, such as personal tax filing, self-employed filing, or business tax services

• Preferred contact method and best time to reach the client are included to improve follow-up efficiency

• Submission confirmation and call-to-action messaging are added to guide next steps, such as scheduling, document preparation reminders, or waitlist updates

Digital Product Policy: This is a digital product. No physical product will be delivered to you. 

Turnaround Time: The typical turnaround time is 5-7 business days after all required information has been received. Business days exclude Saturdays, Sundays, and holidays. Orders placed after business hours will be considered received on the next business day.

Expedited Turnaround: For an additional fee, your order will be prioritized and delivered within 24–48 hours after all required information is received. To avoid delays, please complete our Graphic Design Inquiry Form and submit all details within 2 hours of purchase. The expedited fee is nonrefundable and will be forfeited if information is not submitted on time.

Revision Limit: You are entitled to two revisions to guarantee your full satisfaction. Any additional revisions will be charged $25 each. 

Revision Time: Each revision will take up to 24 hours to complete.
