Our Client Intake Form buildouts are designed to securely collect essential client information while remaining user-friendly, efficient, and professional for tax and financial service providers

• Client information includes full legal name, date of birth, Social Security number, email address, phone number, current home address, filing status, occupation or employer name, and preferred method of contact

• Spouse information, if applicable, includes spouse’s full legal name, date of birth, Social Security number, occupation or employer, email address, and phone number

• Dependent information, if applicable, includes dependent full name, relationship to the client, date of birth, Social Security number, residency status, student status, and childcare expense information. Fields may be repeated for multiple dependents

• Income information includes W-2 income, self-employed or 1099 income, business name if applicable, additional income sources such as rental, investment, retirement, or unemployment income, and additional income notes

• Deductions and credits include education expenses, child tax and childcare credits, student loan interest, medical expenses, charitable contributions, homeownership expenses, and retirement contributions

• Banking and refund information includes preferred refund method, bank name, routing number, and account number

• The document upload section allows secure uploads for W-2s, 1099s, prior-year tax returns, photo identification, Social Security cards if required, and additional supporting documents

• Due diligence questions may be included, as applicable, to support compliance and accurate filing (such as filing status verification, dependent eligibility, and credit/deduction confirmation).

• A Letter of Engagement is attached to the intake form and outlines the scope of services and limitations, client and preparer responsibilities, fees and payment terms (if applicable), consent for e-filing and electronic communication, record retention, confidentiality, and authorization to proceed.

• Authorization and signatures are included through electronic signature fields, along with the submission date and an acknowledgment confirming the information provided is accurate and complete and that the client understands their responsibility for items submitted to any taxing authority.

• Client information includes full legal name, date of birth, Social Security number (if required), email address, phone number, current home address, mailing address (if different), and preferred method of contact

• Identity verification section may include photo identification upload, proof of address (utility bill or lease), and Social Security card upload (if required for compliance purposes)

• Credit profile overview includes current credit score range (if known), number of negative accounts, active collections, late payments, charge-offs, repossessions, foreclosures, bankruptcies, or judgments

• Credit goals section includes client objectives such as home purchase, auto loan approval, business funding, credit card approvals, debt consolidation, or score improvement targets

• Financial snapshot includes current employment status, monthly income range, monthly debt obligations, and open credit accounts

• Existing disputes section allows clients to indicate whether they have previously filed disputes, worked with another credit repair company, or contacted creditors directly

• Creditor and bureau information includes known reporting agencies (Experian, Equifax, TransUnion), account names, account numbers (last four digits only), and dispute details if available

Once you have paid click HERE to complete the design inquiry form. This form must be completed within 2 hours. Failure to do so could delay production of your digital product(s).

Digital Product Policy: This is a digital product. No physical product will be delivered.

Turnaround Time: 4-7 business days and begins once all required information has been received. Business days exclude weekends and holidays.

Expedited Turnaround: Available for an additional fee and delivered within 24–48 hours once all required information has been received. All materials must be submitted within 1 hour of purchase with timely communication. Failure to do so may result in forfeiture of the expedited fee and the order reverting to standard turnaround.

Revision Limit: Includes up to two complimentary revisions within scope. Revisions must be requested within 24 hours of delivery or will be forfeited. Additional revisions or requests outside of scope are $25 each.

Revision Time: Each revision may take up to 24 hours and is not included in the original delivery timeframe.

Communication & Inactivity: Timely communication and submission of required materials are required. Failure to respond may result in delays, project inactivity, or forfeiture of services per our full policy.

Delivery: Delivery is considered complete once final files have been sent.
